This software provides a form that will allow you to record vital information needed in every specialty. Log information pertaining to the patient’s main reason for the visit, brief history of the illness, results from a physical exam, organicity indicators, check from a variety of signs & symptoms when positives result. There are areas for psychiatric exam results, as well as DSM IV diagnosis for Axis I-V and a decision-making recording of planned treatment and/or medications prescribed.
